Abstract
Because of decreased host immunity, the poorly controlled diabetic patients are susceptible to infections of the urinary tract, respiratory tract and skin, etc. The incidence of acute mediastinitis is rare. The usual causes of acute mediastinitis
are
esophageal perforation or median sternotomy during open heart surgery Factors associated with the increased incidence of acute mediastinitis after open heart surgery include the presence of postoperative complications, triple valve replacement,
and
diabetes mellitus. Twenty five percent of acute mediastinitis are non-traumatically developed. Most of them are developed by extension of infections of the head and neck through the cervical fasciae.
We report a 68-year-old woman with NIDDM complicated by acute mediastinitis in whom the retropharyngeal abscess and left lateral neck cellulitis caused by Klebsiella pneumoniae, spreaded to the mediastinum.
